Rates
$125 per 55-minute session

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Dr. Humphrey does not process insurance forms directly.  You pay for his services at the time you receive them.  Dr. Humphrey will provide for you an appropriate receipt which you then submit directly to your insurance provider.  Your insurance provider will reimburse you directly.

Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Payment
Cash or check at the conclusion of your session.

Cancellation Policy
If you know that you are not going to be able to make an appointment, please give Dr. Humphrey 48 hours notice so that he has ample time to fill the session time with another person.  If you simply forget your appointment or choose to do something else and do not give the prescribed 48 hours notice the full fee is still due and payable at your next session.  Dr. Humphrey is open to and reasonable about discussing these issues if extenuating circumstances have arisen (e.g. illness).
